: Under direct supervision, the Surgery / ED / Observation Coder is responsible for accurate coding of professional services, including diagnoses, procedures, and modifiers from medical records in hospital, clinic, outpatient surgery center, urgent care, and emergency department settings. The role involves analyzing medical records and assigning ICD-10-CM, CPT, and HCPCS Level II codes with appropriate modifiers. Medical coding is performed in accordance with ICD-10-CM Official Guidelines for Coding and Reporting, CPT guidelines for professional and surgical services, CMS updates, Coding Clinic articles published by the American Hospital Association, HCPCS guidelines, NCCI edits, LCD/NCD determinations, and client-specific coding guidelines. EXPERIENCE: Minimum 0 to 5 years of experience in multispecialty Surgery, Observation, Emergency Department (ED), and Evaluation & Management (E&M;) medical coding. EDUCATIONAL QUALIFICATION: Any Graduate is fine. Life Science or Allied Medicine Graduates with certification from AAPC or AHIMA. PROFESSIONAL QUALIFICATION:
- Hands-on experience in coding multispecialty Surgical, Observation, Emergency Department, and Critical Care services.
- Experience in specialties such as Dermatology, Gastroenterology, Cardiology, Otolaryngology, and other multispecialty outpatient services.
- Robust understanding of Evaluation & Management (E&M;) coding and Medical Decision Making (MDM).
- Sound knowledge of Medical Terminology, Human Anatomy, and Physiology.
- Proficient in ICD-10-CM, CPT, Modifier, and HCPCS coding guidelines.
- Expertise in applying modifiers as per CPT and NCCI edit guidelines. ROLES AND RESPONSIBILITIES:
- Verify and abstract all relevant information from medical records to assign accurate diagnosis and procedure codes for outpatient surgery centers, hospitals, urgent care centers, and emergency department settings.
- Review and analyze documentation to assign appropriate ICD-10-CM, CPT, HCPCS, and modifier codes.
- Determine correct CPT codes for surgical procedures, E&M; levels, vaccines, laboratory services, screening services, PQRS codes, and minor procedures based on documentation.
- Apply modifiers appropriately in compliance with CPT guidelines and NCCI edits.
- Review LCD and NCD coverage determinations and modify ICD-10-CM, CPT, and modifier usage accordingly.
- Maintain productivity and quality standards with the ability to code both surgical and ED charts efficiently.
- Stay updated with AMA, AHA, CMS, and other regulatory coding guideline changes and implement them effectively.
- Organize and prioritize work to ensure coding completion and edit resolution within required turnaround times and compliance standards.
- Work collaboratively as a team player by contributing to process improvement, productivity enhancement, and quality initiatives.
- Ensure adherence to HIPAA policies and maintain confidentiality and security of patient records and organizational systems.
